Job Description

Summary

This position provides comprehensive case services to all clients who are eligible for assistance through the Houston Recovery Center Programs Duties includes: SUD intake and assessment, individual service plans, transportation, weekly case management, services coordination, progress monitoring and reporting, advocacy, and referrals.

 

This position is located Houston Recovery Center 150 N Chenevert St, Houston, TX 77002.

 

The ideal candidate will have knowledge of community resources agencies including adult substance use treatment facilities and their services.  Will also possess excellent communication skills and able to work cooperatively with staff.

Job Duties

  • Provides intake, assessments and referral for clients in need of substance abuse services. 
  • Provides client transportation to different levels of care.
  • Provides comprehensive case services to clients of the Houston Recovery Center enrolled in its programs.
  • Assist the client identify his/her desired goals and objectives to design an Individual Service Plan customized to the client needs.
  • Connect clients with needed and available community resources and facilitate client's transfers between different levels of care, avoiding gaps in services. 
  • Effectively manages a clients case load, meeting with clients daily or weekly according to client's level of care.
  • Participates in weekly department case consultation staffing. 
  • Participates in regular staff meetings, staff training programs and accept the responsibility for aiding the development of positive team relationships as requested.
  • Adheres to agency policy, procedures and the professional code of ethics.

Minimum Qualifications

  • Bachelor's degree in Social Work, Psychology, Human Services or a related field.
  • Three years of relevant experience.

Preferred Qualifications

  • Certified as Recovery Support Specialist by Texas Board of Addition Professionals.
  • Licensed Chemical Dependency Counselor.
